Oklahoma Program Information
What are the eligibility requirements?
- You must be 55 years or older, legally blind and want to maintain or regain greater independence.
- Service eligibility is not income-based.
- Eligibility is based on significant vision loss using the definition for legal blindness. (Legal blindness occurs when visual acuity with best correction is 20/200 or less or when side vision results in a field restriction of 20 degrees or less.)
- Eligibility can also be based on the latest report from an eye doctor, the report is to be from within the past 12 months.
- An in-person vision assessment is conducted, and functional limitations questions are asked. Based on the eye report, vision assessment, and functional limitations questions, a decision for eligibility for services is determined.
What is the process for applying for services?
- Step 1: Call for an appointment
- Step 2: An office staff member will help you fill out an application
- Step 3: If you need a ride the office will make arrangements to pick you up and take you to the office.
What documentation is required?
- A picture ID (driver's license, state ID or military ID),
- Insurance/Medicare card, and
- Your most recent eye report (if you have it).

- Rehabilitation Teaching: Dialing the phone, signing your name, cooking, identifying money, telling time, matching your clothing, and more!
