Alabama Program Information

What are the eligibility requirements?

Consumers must be 55 and over, a legal resident of Alabama, and have a significant visual impairment that affects a person’s ability to perform one or more daily life activities. Our staff conduct and individualized functional assessment to determine if the person meets these requirements. 

What is the process for applying for services?

Call to be placed on the referral list in your local area. Staff will arrange an individualized assessment, usually at your home. During the assessment, they will also complete our paperwork for applying for services. 

What documentation is required?

No documentation is required, but medical documentation is preferred to learn more about the consumer's eye condition. Our staff can work with a consumer’s eye care provider to get medical records to gain information on a person's vision impairment. 

What services are available?

  • In-person assessment at the consumer's home to learn more about their vision loss and the activities of daily living that it is affecting
  • Instruction on independent living skills from a Vision Rehabilitation Therapist that may involve learning adaptive aides or techniques
  • Evaluation in low vision aides and devices
  • Training in navigating the home and Community from Orientation and Mobility Specialists
  • Referral to agencies and programs that can benefit the consumer, including local and virtual peer support groups

How do I get connected with the program?

  • We take referrals from anyone – eye care providers, state agencies, non-profits, friends, family, or self-referrals. Potential consumers can call the main office number or call a local office number.
