What are the eligibility requirements?

Eligibility for services is established by an individual’s visual acuity as determined by their eye doctor.

How do I apply for services?

An application to DVI for services can be initiated via phone call by any individual who is either visually impaired themselves or is aware of a visually impaired individual. 

What documentation is required?

Minimal information is needed to start the application process, such as name, address, phone number, and date of birth. Eye report with visual acuity and etiology.
