What are the eligibility requirements?

You must have had an eye exam in the past two (2) years from an ophthalmologist or optometrist.

How do I connect with the program?

Go to Utah's Services for the Blind and Visually Impaired to connect with a representative. You can also connect with Centers for Independent Living (CILs), or an ophthalmologist or optometrist can give you information.

What is the process for applying for services?

  1. Submit LV Referral
  2. Contact DSBVI at 801-323-4369
  3. Contact an OIB Specialist in the CIL’s

What documentation is required?

Low Vision Referral Patient’s Eye Report – DWS -USOR form 149 

What services are available?

  • Low Vision evaluations/clinics
  • Assessments for low vision devices and aids, for near and distance viewing
  • Assessments on non-optical devices and products
  • Help to improve lighting and reduce glare
  • Training on assistive technology, on-line apps, computer accessibility features and mobile devices
  • Vision training and therapy
  • Home visits in specific situations as needed
