What are the eligibility requirements?
You must have had an eye exam in the past two (2) years from an ophthalmologist or optometrist.
How do I connect with the program?
Go to Utah's Services for the Blind and Visually Impaired to connect with a representative. You can also connect with Centers for Independent Living (CILs), or an ophthalmologist or optometrist can give you information.

What is the process for applying for services?
- Submit LV Referral
- Contact DSBVI at 801-323-4369
- Contact an OIB Specialist in the CIL’s
What documentation is required?
Low Vision Referral Patient’s Eye Report – DWS -USOR form 149
What services are available?
- Low Vision evaluations/clinics
- Assessments for low vision devices and aids, for near and distance viewing
- Assessments on non-optical devices and products
- Help to improve lighting and reduce glare
- Training on assistive technology, on-line apps, computer accessibility features and mobile devices
- Vision training and therapy
- Home visits in specific situations as needed
